What Is Pulse Pressure?
While most people focus on their systolic and diastolic numbers separately, pulse pressure provides additional information about arterial health that neither number reveals on its own. Research published in leading cardiology journals has consistently shown that wide pulse pressure is an independent predictor of heart attack, stroke, heart failure, and overall cardiovascular mortality.
What Is Mean Arterial Pressure (MAP)?
Mean arterial pressure is the average pressure in your arteries throughout the entire cardiac cycle — not just during the heartbeat (systolic) or between beats (diastolic). It is calculated as: MAP = Diastolic BP + (Pulse Pressure ÷ 3).
MAP is the pressure that actually drives blood to your organs. Clinicians use MAP rather than systolic pressure when assessing whether organs are receiving adequate blood flow. A MAP below 60 mmHg is considered critically low — at this level, the kidneys, brain, and heart may not receive sufficient perfusion.
What Causes Wide Pulse Pressure?
Wide pulse pressure (above 60 mmHg) most commonly indicates arterial stiffness — the arteries have lost their normal elasticity and cannot absorb the pressure wave of each heartbeat as effectively. This causes the systolic number to rise while diastolic may remain normal or even fall.
Common causes include:
- Age-related arterial stiffening — the most common cause in adults over 50
- Atherosclerosis — plaque buildup that hardens artery walls
- Isolated systolic hypertension — systolic elevated, diastolic normal; very common in older adults
- Aortic regurgitation — leaking aortic valve causes very wide pulse pressure
- Severe anaemia — low red blood cell count increases cardiac output
- Hyperthyroidism — elevated thyroid hormones increase cardiac output
- Bradycardia — slow heart rate compensated by larger stroke volume
What Causes Narrow Pulse Pressure?
Narrow pulse pressure (below 25 mmHg) can indicate reduced cardiac output. Causes include:
- Heart failure — the heart cannot pump effectively, reducing the difference between systolic and diastolic
- Cardiac tamponade — fluid around the heart restricting its ability to fill
- Severe aortic stenosis — narrowed aortic valve limits blood ejection
- Significant blood loss or severe dehydration
- Shock of any cause
A suddenly narrow pulse pressure in someone with previously normal readings warrants urgent medical evaluation.
How to Reduce Pulse Pressure
Reducing pulse pressure requires improving arterial elasticity and reducing the workload on the heart. Evidence-based approaches include:
- Regular aerobic exercise — particularly endurance exercise (walking, swimming, cycling) which directly improves arterial compliance
- Low-sodium diet — reducing sodium intake lowers systolic pressure, which directly reduces pulse pressure
- Mediterranean diet — associated with improved arterial health and reduced cardiovascular risk
- Achieving and maintaining a healthy weight — excess body weight increases arterial stiffness
- Stopping smoking — smoking is one of the most potent causes of accelerated arterial stiffening
- Limiting alcohol — excess alcohol raises blood pressure and worsens arterial health
- Managing stress — chronic stress raises cortisol, which contributes to hypertension and arterial damage
- Blood pressure medications — some classes (ACE inhibitors, ARBs) have been shown to specifically improve arterial compliance beyond their blood pressure-lowering effect
Pulse Pressure and Age
Pulse pressure increases with age as a natural result of arterial stiffening. For this reason, age-adjusted interpretation is essential. A pulse pressure of 65 mmHg in a 75-year-old carries a different risk profile than the same reading in a 35-year-old, where it would be more unusual and concerning.
In older adults, isolated systolic hypertension — high systolic pressure with normal diastolic pressure — is the most common form of hypertension and the most common cause of wide pulse pressure. Treatment of isolated systolic hypertension in older adults has been shown to reduce stroke and heart attack risk significantly.
