What Are B Vitamins and Why Do You Need All 8?
Although the B vitamins are grouped together and often sold as a "B complex," each one has distinct functions, distinct food sources, and distinct deficiency consequences. They cannot substitute for each other. This is why a comprehensive assessment of all eight is more informative than checking just one or two.
All B vitamins are water-soluble, meaning the body cannot store large reserves (with the significant exception of B12, which is stored in the liver for years). This means a consistent daily supply is needed, and also that excess amounts are generally excreted safely in urine — making toxicity rare at standard doses.
B12 — The Most Critical Deficiency Risk
Vitamin B12 (cobalamin) is perhaps the most important B vitamin from a public health perspective, because its deficiency is both common and potentially devastating. B12 is found almost exclusively in animal products — meat, fish, dairy, and eggs. Plant foods contain essentially no active B12 unless fortified.
This makes vegans and strict vegetarians at extremely high risk of B12 deficiency. The consequences are serious: B12 deficiency causes megaloblastic anaemia, peripheral neuropathy (nerve damage causing tingling and numbness), subacute combined degeneration of the spinal cord, cognitive impairment, and severe psychiatric symptoms. Critically, some of the neurological damage from B12 deficiency can be irreversible if not treated promptly.
What makes B12 deficiency particularly dangerous is its slow onset. The liver stores 2–5 years' worth of B12, so vegans who do not supplement may feel fine for years before symptoms begin — by which point, significant neurological damage may have already occurred. This is why B12 supplementation is non-negotiable for vegans and strongly advised for vegetarians. If you are vegan, please do not rely on nutritional yeast as your sole B12 source unless it is specifically fortified — most nutritional yeast does not contain reliable active B12.
For people over 50, B12 absorption from food also declines significantly because the stomach produces less intrinsic factor — the protein needed to absorb B12. Crystalline B12 in supplements and fortified foods bypasses this problem and is absorbed by a different mechanism, making supplementation increasingly important with age. Folate (B9) — Critical Before and During Pregnancy
Folate (vitamin B9) plays a central role in DNA synthesis, cell division, and the production of red blood cells. Its most critical function is supporting the development of the neural tube in the early weeks of pregnancy — the structure that becomes the brain and spinal cord. Inadequate folate in the weeks before conception and in early pregnancy is a leading cause of neural tube defects, including spina bifida and anencephaly.
The NHS, CDC, and virtually every major health organisation in the world recommend that all women who are planning a pregnancy or could become pregnant take 400 mcg of folic acid daily, starting at least one month before conception and continuing through the first 12 weeks of pregnancy. Women with a previous neural tube defect pregnancy are advised to take a higher dose (5mg daily) under medical supervision.
Beyond pregnancy, folate deficiency causes megaloblastic anaemia, elevated homocysteine levels (a cardiovascular risk factor), and impaired immune function. B6 — More Than Just Metabolism
Vitamin B6 (pyridoxine) is involved in over 100 enzyme reactions, primarily related to protein metabolism. But its role extends far beyond digestion: B6 is needed to synthesise neurotransmitters including serotonin, dopamine, and GABA — the brain's primary inhibitory neurotransmitter. This makes B6 deeply important for mood regulation, sleep quality, and anxiety management.
B6 is also needed for melatonin production, the hormone that regulates the sleep-wake cycle. B6 deficiency has been linked to insomnia and disrupted sleep patterns. Oral contraceptive pills significantly deplete B6 in women — one of the most important medication-related B vitamin interactions. Women on the pill who experience mood changes, low energy, or PMS symptoms may benefit from monitoring their B6 status.
Thiamine (B1) and Alcohol Depletion
Thiamine (B1) is essential for carbohydrate metabolism and nerve function. Severe B1 deficiency causes Wernicke's encephalopathy — a serious neurological emergency characterised by confusion, eye movement abnormalities, and difficulty walking. This condition is most commonly seen in people with heavy alcohol dependence, as alcohol dramatically reduces thiamine absorption and depletes thiamine stores.
Even moderate regular alcohol consumption impairs the absorption of thiamine, riboflavin, folate, and B12. This is one of the most important reasons why people who drink alcohol regularly should pay particular attention to their B vitamin intake. Alcohol's effect on B vitamin status also partly explains the fatigue, mood changes, and cognitive difficulties associated with regular drinking beyond just the direct effects of alcohol itself.
Medications That Deplete B Vitamins
Several commonly prescribed medications significantly reduce B vitamin levels:

- Proton pump inhibitors (PPIs) — omeprazole, lansoprazole, and similar drugs reduce stomach acid, which is needed for B12 absorption from food. Long-term PPI use is associated with B12 deficiency.
- Oral contraceptive pill — depletes B6, B9 (folate), and B12 over time.
- Anticonvulsants — many antiepileptic drugs impair folate absorption and metabolism, and some also affect B6 and B12.
- Methotrexate (used in arthritis and some cancers) — directly antagonises folate metabolism; supplemental folic acid is routinely prescribed alongside it.
Why "Always Tired" Can Be a B Vitamin Issue
Fatigue is the most common symptom of B vitamin deficiency, and it is understandable why: B vitamins are the backbone of cellular energy production. B1, B2, B3, and B5 are all directly involved in the mitochondrial processes that convert carbohydrates, fats, and proteins into ATP — the cell's energy currency. B12 deficiency causes anaemia that reduces the oxygen-carrying capacity of the blood, and the brain's energy demands are particularly sensitive to B12 status.
