How Much Calcium Do You Really Need?
Calcium is the most abundant mineral in the human body. About 99% of your body's calcium is stored in bones and teeth, where it provides structural strength. The remaining 1% circulates in the blood and soft tissues, where it plays essential roles in muscle contraction, nerve transmission, blood clotting, and cell signalling.
What many people do not realise is that the body maintains blood calcium within a very narrow range. If dietary calcium is insufficient, the body simply dissolves calcium from bone to maintain blood levels. This process — when sustained over years — gradually weakens bones and sets the stage for osteoporosis. The bones do not complain as they thin; there are no symptoms until a fracture occurs.
The Critical Link Between Calcium and Vitamin D
You can eat all the calcium in the world and still be deficient if your Vitamin D levels are inadequate. Vitamin D is essential for calcium absorption in the small intestine. Without sufficient Vitamin D, the body absorbs only 10–15% of dietary calcium. With optimal Vitamin D levels (above 75 nmol/L), absorption rises to 30–40%.
This is why calcium and Vitamin D are almost always recommended together. And it explains a paradox that puzzles many researchers: populations with relatively high dairy intake sometimes still have high rates of osteoporosis — partly because they also have high rates of Vitamin D deficiency, particularly in northern countries with limited sunlight. Building Peak Bone Mass — Why Early Life Matters Most
Bone density peaks in your late 20s to early 30s. After that, bone loss begins to exceed bone formation, gradually reducing bone density with age. The higher your peak bone mass, the more bone you can afford to lose before reaching the osteoporotic threshold.
This is why calcium intake during childhood, adolescence, and early adulthood is so important. Teenagers (particularly girls) need 1,300mg of calcium daily — more than adults — because this is the critical window when bones are most actively mineralising. Research shows that calcium intake during adolescence is one of the most important determinants of lifelong bone health and fracture risk in later life.
By the time osteoporosis is diagnosed, significant bone loss has already occurred. Prevention — through adequate calcium, Vitamin D, weight-bearing exercise, and avoidance of smoking and excessive alcohol — is far more effective than treatment.
The Best Non-Dairy Calcium Sources
Dairy products are the most concentrated and bioavailable source of calcium in the Western diet, but they are far from the only source. For people who are vegan, dairy-free, or lactose intolerant, meeting calcium needs is entirely achievable with the right food choices.
- Fortified plant milks — soy, oat, almond, and rice milks are typically fortified to contain 280–400mg per cup, comparable to cow's milk. Shake the carton well, as calcium settles at the bottom.
- Canned fish with bones — sardines (325mg per serving) and canned salmon with bones (180mg) are excellent sources where the soft, edible bones are the calcium-rich component.
- Tofu made with calcium sulfate — check the label; tofu set with calcium sulfate contains 200–400mg per half cup. Nigari-set tofu contains much less.
- White beans and legumes — white beans (130mg per half cup), soybeans, and chickpeas provide meaningful calcium alongside fibre and protein.
- Leafy greens — cooked collard greens (270mg per cup), kale (100mg), and pak choi (160mg) provide well-absorbed calcium. Note that spinach, despite being high in calcium, also contains oxalate which binds calcium and reduces absorption — it is not a reliable calcium source despite its reputation.
- Almonds — 75mg per oz, plus healthy fats and Vitamin E.
- Fortified orange juice — 300mg per cup, with the bonus of Vitamin C.
- Chia seeds — 180mg per oz, with omega-3 fatty acids.
Note that calcium from plant sources is often less bioavailable than from dairy, partly because of compounds like oxalate and phytate that bind calcium. However, absorption from fortified plant milks and low-oxalate greens like kale is comparable to milk. It is important to pair calcium-rich foods with adequate Vitamin D for optimal absorption. Avoid drinking tea or coffee with calcium-rich meals as tannins can reduce absorption.
What Depletes Calcium From Your Body
Meeting your calcium intake target is only half the equation. Several common dietary and lifestyle factors actively increase calcium losses, effectively reducing how much your body retains.
High sodium intake is the biggest calcium thief in the modern diet. For every gram of excess sodium excreted, approximately 26mg of calcium is lost in the urine. Given that most adults eat 2–4 grams more sodium than recommended daily, this adds up to 50–100mg of extra calcium loss every single day. Excessive caffeine modestly increases calcium loss. Heavy coffee and tea consumption (more than 4 cups daily) increases urinary calcium excretion by approximately 6mg per cup. This is manageable for most people with adequate calcium intake, but becomes significant in those who are already borderline deficient.
Smoking reduces calcium absorption and impairs bone formation. Smokers have significantly lower bone density and higher fracture rates than non-smokers, independently of other risk factors.
Excessive alcohol impairs calcium absorption, reduces Vitamin D activation in the liver, and directly inhibits bone-forming osteoblast cells. More than 2 drinks per day consistently is associated with lower bone density.
Very high protein intake — particularly from animal sources — has been shown to increase urinary calcium excretion. However, the evidence is nuanced: protein is also important for bone health. The key is maintaining adequate calcium alongside higher protein diets rather than restricting protein.
Calcium Supplements — Carbonate vs Citrate
If dietary calcium is insufficient, supplements can fill the gap. The two most common forms are calcium carbonate and calcium citrate, and choosing between them matters.
Calcium carbonate is the most widely available and cheapest form. It contains 40% elemental calcium by weight, meaning a 500mg tablet provides 200mg of actual calcium. However, it requires stomach acid for absorption and must be taken with food. It is the best choice for most people under 50 with normal stomach acid production.
Calcium citrate contains only 21% elemental calcium but can be taken without food or stomach acid, making it far superior for people over 50 (who produce less stomach acid), those taking proton pump inhibitors or antacids, or anyone with digestive issues. It is more expensive but more reliably absorbed in older adults.
Regardless of form, never take more than 500–600mg in a single dose — the body cannot absorb larger amounts at once. Osteoporosis — The Silent Epidemic
Osteoporosis affects an estimated 200 million women worldwide and is responsible for approximately 8.9 million fractures annually. In the US, 10 million people have osteoporosis and another 44 million have low bone density. In the UK, one in two women and one in five men over 50 will have a fracture due to osteoporosis in their lifetime.
The condition is called a "silent disease" because bone loss occurs without symptoms. Many people discover they have osteoporosis only after a fracture — often from a relatively minor fall that would not have broken a healthy bone. Hip fractures in older adults carry serious consequences: approximately 20% of people who suffer a hip fracture die within a year, and many others permanently lose independence.
Prevention starts decades before osteoporosis would develop. Building maximum peak bone mass in youth, maintaining it through mid-life with adequate calcium, Vitamin D, and weight-bearing exercise, and minimising bone-depleting habits (smoking, excess alcohol, high sodium) all contribute to long-term bone health.
