What Is Waist to Hip Ratio?
Unlike BMI, which measures total body weight relative to height, WHR specifically measures where your body stores fat. Central fat (stored in the abdomen) is metabolically very different from peripheral fat (stored in hips and thighs). This distinction matters enormously for health risk assessment.
Why Fat Distribution Matters More Than Total Weight
Two people can weigh exactly the same and have the same BMI, yet have completely different health risk profiles depending on where their fat is stored. Visceral fat (stored in and around the abdominal organs) releases inflammatory molecules, disrupts insulin signalling, raises blood pressure, and increases LDL cholesterol. Subcutaneous fat stored in the hips and thighs is largely metabolically inert by comparison.
This is why the "apple shape" (high WHR) carries significantly higher risk than the "pear shape" (low WHR), even at the same total body weight.
Apple vs Pear Shape — Health Implications
Apple shape (high WHR) — Fat stored in the abdomen. Associated with:
- Higher risk of type 2 diabetes (2-3x increased risk)
- Higher risk of cardiovascular disease and heart attack
- Increased risk of stroke
- Higher blood pressure and cholesterol levels
- Greater risk of metabolic syndrome
- Increased risk of certain cancers (particularly colon and breast)
Pear shape (low WHR) — Fat stored in hips and thighs. Associated with:
- Lower cardiovascular risk than apple shape at the same body weight
- Some research suggests a protective effect from hip fat due to anti-inflammatory adipokines
- Lower risk of type 2 diabetes and metabolic syndrome
WHR vs BMI vs Waist Circumference
BMI — does not distinguish between muscle and fat or account for fat distribution. Athletes can have high BMI with low health risk; sedentary people can have normal BMI with dangerous abdominal fat.
Waist circumference alone — does not account for body size. A waist of 90 cm carries different implications on a 5 ft person than a 6 ft 2 in person.
WHR — adjusts for body size by comparing waist to hips. Multiple studies have shown WHR outperforms both BMI and waist circumference alone in predicting cardiovascular events and mortality.

How to Reduce Your WHR
- HIIT and aerobic exercise — most effective at targeting visceral abdominal fat specifically
- Strength training — builds muscle mass, raises resting metabolic rate, and improves fat distribution
- Reduce refined carbohydrates and sugar — insulin spikes promote abdominal fat storage
- Limit alcohol — strongly promotes visceral fat accumulation (the "beer belly" effect)
- Manage stress — chronic cortisol elevation directly promotes abdominal fat storage
- Improve sleep — sleep deprivation raises cortisol and promotes central fat accumulation
- Mediterranean diet — specifically shown to reduce visceral fat in clinical trials
How WHR Changes With Age
WHR tends to increase with age as metabolism slows and hormones change. In women, the transition through perimenopause and menopause is particularly significant — oestrogen decline shifts fat distribution from the hips and thighs toward the abdomen, often changing the body shape from pear to apple. This is normal but not inevitable with exercise and diet.
Men tend to accumulate abdominal fat more readily than women in younger years (testosterone partly suppresses hip fat storage), so their WHR risk is assessed at a lower ratio threshold.
